The main focus of Community Access Support is to enhance the quality of life, social interaction and ability to access the general community of a person living within that community. The person may develop choice, independence, self-responsibility and self-reliance in the general community. We require participants to work in cooperation with the aims of the program, and to have the desire to increase social skills as a major contributing factor to their own independence. Each person accessing the Community Access program will receive a service designed to meet (in the least restrictive way) his or her individual needs and personal goals and will have the opportunity to participate as fully as possible in the decision making process regarding events and activities in their life, particularly in relation to the service they receive.

The Post School Services - Adult Lifestyle Support Program (formally known as the Moving Ahead Program) is one of this range of funding and support programs and is for school leavers who have a disability resulting in high and complex support needs.
The Post School Services Program assists young people to make the transition from school to adult life and supports them to develop skills that enable them to participate in the life of their local community and take on adult roles within their families. The Post School Services Program also supports families to plan for the young person's future and identify informal and formal networks needed to help them achieve their goals. Our Post School Services program aims to enhance those skills learnt throughout school and assist in a smooth transition from school life into life within the general community. It is aimed at school leavers with complex or high support needs. We recognise that all of our clients have individual support needs and therefore require the development and implementation of flexible, innovative and individualised support plans to achieve outlined goals. We recognise the important roles that families play in the implementation of the initial stages of this future plan as they often hold they key to providing support in a style most suited to that person. In saying this, we encourage all involved persons to assist us in matching staff who will provide support in a manner that will complement and reflect our clients' abilities. It is wonderful to watch our clients take the steps towards meeting their desired goals. We have supported clients to achieve increased self-esteem, develop communication skills, develop confidence, increase social networks, build friendships, participate in community life, complete TAFE courses, liaise with other agencies to gain employment and training, increase independent living skills to a point where they can consider alternative living arrangements and make a valuable contribution to their family. The goals that have been achieved by our clients must be accredited to their own efforts and the valuable input of their families and the support provided by our support staff.
Our approach in providing quality support provision in a supportive and open environment is the success behind our program. The openness encourages families and clients to liaise with staff members freely and feel comfortable within our relaxed environment.
Attendant Care is assistance in carrying out those activities of daily living which, because of functional limitations, individuals are unable to adequately perform themselves. This support may include assistance with household management, mobility, transfers, personal hygiene, bathing, grooming, dressing, eating, drinking and similar personal activities. This scheme is intended to assist people with functional limitations caused by a physical impairment, although clients may also have sensory or mental impairments. However, clients must be able to exercise control over their environment and to direct and supervise their attendant carers (where the disability presents difficulties such as severe speech impairment, the client may use a friend or advocate to direct attendance if they wish). The scheme is intended to cater for people with a disability of working age, i.e. between 16 and 64 years of age.

The Adult Lifestyle Support Program is directed by our clients in receipt of this funding package. Clients within this program have support needs that differ to those within other programs. The support that we provide enables our clients to live comfortably in accommodation of their choice, gain employment by referring to specialised agencies, maintain family relationships, develop internal and external networks, integrate and become involved in community activities and complete day to day living activities. We recognise that the level of support required by clients within this program is often variable dependent upon what is happening in their life at any given time. We accommodate this by providing staff that are trained in identifying signs that may be indicative of a required change in support delivery. Clients in receipt of our Adult Lifestyle Support Program often request support that requires an amount of lateral thinking. Our flexible approach enables these requests to be easily met by creating and implementing support activities, and engaging agencies with whom we have effective working relationships.
The Family Support Program for adults provides flexible and responsive support to families who have high and complex needs. These supports are designed to strengthen their ability to care for their adult family member with a disability as well as continue to care for all the family. The aims of family support are: - To strengthen the capacity of families to care for family members with a disability
- To assist families to build formal and informal supports
- To assist families to increase their knowledge, skills and confidence
- To support the inclusion of family members with a disability into community life
- To minimize the incidence of crises in families
We provide support to individuals in receipt of Family Support funding in a manner that provides maximum benefits to the individual as well as the family unit. The support aims to strengthen family relationships by providing a form of respite to all involved thereby avoiding incidences of crises and an ability to strengthen their capacity to care for the family member with a disability. We assist in developing formal and informal networks that can assist outside of the program, and we support and encourage inclusion into community activities. This program aims to meet the needs of individuals that are not currently being met through other programs. Our staff work closely with the family to identify where support is required most, and to provide support that will benefit the family the most.
